users and we are committed to protect the information collected through our fitness & wellness
application. Information such as but not limited to, Quarterly Questionnaire, Lifestyle Questions,
Fitness, Stretching, Yoga, Physical Therapy, Meditation, Medical Concierge center, Drug &
Alcohol Center, On Site Wellness Days etc (any or all of which are referred to as “Apps”We
adhere to ethical standards in gathering, using, and protecting any information that was provided
to us. OTM Training has acquired this private policy “(Private Policy)” to explain how OTM
Training’s application collects, stores and uses the information collected to help generate
reporting to employers. By using this application and registering your information, you agree to
this private policy and give explicit information and approve consent to the usage, storage and
the processing of your personal data in accordance with this private policy. If you do not agree to
this privacy policy, please do not download and install this application


User Private Information

When you download the application or use the website along with other products and services,
your information will be obtained. Upon registration you generally provide us with your name,
age, work email address, phone number, location, and generate a password, your provided
information when filling out the “contact us” form. Information collected from users can be
categorized into “Personal information,” “Sensitive personal information,” and “Associated
information.” International Data Protection rules will be followed treating and safeguarding all
information collected by the Application or Website. Your provided information may be used to
contact you from time to time, to provide you with new services, important information, blogs,
new released videos and marketing promotions. You will be asked when you need more
information that personally identifies you or allows OTM Training to contact you. In order for
you to make the best use of the Application and enable OTM Training to capture your
information accurately, it is vital that you log in using your own credentials.

OTM Training’s Application may collect certain information automatically, including and not
limited to, the type of mobile device you use, your mobile devices unique device ID, the IP
address of your mobile device, your mobile operating system, the type of mobile internet
browsers you use, and information about the way you use the Application. The relevant
information may be collected as per the permissions that you provide. Some of our services or
products may require camera and microphone access while you evaluate such services to
improve the service or product experience.

research, administer the application, understand each user’s learning patterns and movements
around the application. Aggregated and individual, anonymized and non-anonymized data may

be in an aggregate form and does not contain any of your Personally Identifiable information.

personal data. (3) correction of your personal data if it is inaccurate; or (4) deletion or restriction
of your personal data in certain circumstances provided by applicable law. These rights are not

Access To Your Personal Information
OTM Training will ensure that your Personal Information is correct and current. If you have
filled out a user profile, you will be able to view your answers that you provided. User will be
able to change their password but will not be able to change their personal information such as
their company email, which would need to be tracked back with their employer. There will be
security measures to ensure your password is protected from being exposed or disclosed. We
collect additional personal data from you to enable particular features within our Platform. For
example, we request access to your phone’s location data to log your run route. This personal
data includes your: movement data from your device’s accelerometer & sensor data, including
heart rate and (GPS) location data. When interacting with our Platform, certain data is
automatically collected from your device or web browser. More information about these
practices is included in the “Cookies” section of this privacy policy below. This data includes:
Device IDs & network access; and Cookies, IP addresses, referrer headers, data identifying your
web browser and version,interactions with our Platform.

are used to track your preferences, help you login faster, and aggregated to determine user trends.
This data is used to improve offerings such as providing more content in areas of greater interest

OTM Training may provide links and reporting in a specific format that enables them to keep
track of video count, views, and popularity through the application. This information is used to
improve customized content as well as generating reporting to their Employer.
Alerts
OTM Training may alert you by email or phone to inform you the following: any newly released
videos, gauging your enjoyment of the app, partnerships with heart rate monitors or equipment,
blogs, and stats about your health etc. This information which may feel useful to you, through
the Company, has factual information about its subsidiaries/affiliates.
Public Forums
Personal information such as height, weight, BMI, gender, birthday, doctor visits, prescriptions,
stress levels, energy levels, etc will be available to all users, and will be in the beginning set up.
Such information will be filled at your own risk. OTM Training is Compliant with HIPAA. You
are able to choose “I prefer not to answer” to these questions. If you use OTM Training’s
Platform to track your fitness activity or physical characteristics, we will collect this personal
data and store it so that you can review it in the Platform. Your fitness activity data may include
data you enter about your activity or data collected by your device during your activity such as
location data and movement data. We may use this activity data to calculate further information
about your activity, such as distance run, or calories burned, so that the calculated information
can be provided to you as part of the functionality of the Platform.


Securities

It is in the best interest and effort of OTM Training to safeguard the confidentiality of user’s
information. Safeguarding will be in effect at all times and will be provided in order to protect
every information processed and maintained. Employees will have access to this application and
will be provided with a company code by their Employer. We use a variety of technical and
organizational security measures, including encryption and authentication tools, to maintain the
safety of your personal data.

How Long Do We Retain User Data?
User data will be retained for as long as an account is active and as long as you are employed
with your current Employer. This practice may change according to legal and business
requirements.
